ALL PARTICIPANTS MUST COMPLETE AND SUBMIT THE FOLLOWING FORMS:

FLOAT TEST LINK To participate in most classes at Mt. Baker Rowing and Sailing Center, you’ll need to complete a brief float test to ensure your comfort and safety in the water. The test takes about 10 minutes and simulates real-world paddling conditions. Please print out the linked form, and bring to any pool with a lifeguard to complete a float test. This paper form must be turned into the office prior to starting any program.

  • What to Wear: You’ll perform the test wearing pants and a long-sleeve shirt.
  • The Test:
    • Float or tread water for 9 minutes.
    • In the final minute, you’ll put on a life jacket provided by a certified lifeguard.

The float test is valid for three years. Contact the Mt. Baker Rowing and Sailing Center office to confirm your float test status or for assistance with scheduling.

RISK & RELEASE LINK: Filling out the Risk & Release form is quick and easy! Here’s what you need to know:

  • Required for Everyone: All participants, regardless of age or activity, must complete this form.
  • Annual Renewal: Forms expire at the end of each calendar year to ensure everything stays current.
  • Parental Approval for Minors: If you’re under 18, a parent or guardian must sign off before you can submit.
  • Hassle-Free Submission: This online form is simple to complete—just fill it out, and it’s automatically sent to our office.

Make sure to complete your Risk & Release form before participating in any activities.

YOUTH MEDICAL FORM LINK: This form is mandatory for all junior crew athletes and must be completed before participating in practice.

  • Annual Renewal: To keep records current, this form must be updated each year and expires at the end of the calendar year.
  • Convenient Online Submission: Simply complete the form online, and it’s automatically sent to our office—no printing or additional steps required!

YOUTH CODE OF CONDUCT LINK: The Youth Code of Conduct form outlines the expectations and responsibilities for all participants in our programs. By signing this form, athletes commit to fostering a positive, respectful, and inclusive environment both on and off the water. This agreement helps ensure a safe and supportive community for everyone involved.
